Nondiscrimination and Language Help

Help for people with disabilities. We provide reasonable modifications, and appropriate auxiliary aids and services, free of charge and in a timely manner, when they’re needed for an equal opportunity to take part in our care. That includes qualified sign language interpreters and written information in other formats such as large print, braille or accessible electronic formats.

Language help. We provide language assistance free of charge and in a timely manner, including qualified oral interpreters and written translations, when needed for someone with limited English proficiency to understand and take part in their care.

How to get help. Tell your Axis clinician or the facility nurse that you need an interpreter, another format, or a change in how we provide care, or contact our Section 1557 Coordinator:

If you believe we’ve discriminated against you, you can file a grievance with our Section 1557 Coordinator in person, by mail, by phone or by email. We’ll help you file it if you’d like. We’ll look into it promptly and send you a written answer, usually within 30 days. We won’t retaliate against anyone for filing a grievance or complaint.

You can also file a civil rights complaint with the U.S. Department of Health and Human Services, Office for Civil Rights, online at ocrportal.hhs.gov, by mail at 200 Independence Avenue SW, Room 509F, HHH Building, Washington, DC 20201, or by phone at 1-800-368-1019 (TDD 1-800-537-7697).
