This Privacy Policy explains how Axis Multispecialty Group, PLLC (“Axis,” “we,” “us”) collects, uses and shares information through our website, axismultispecialty.com, and through the forms and emails connected to it. Use of the website is also governed by our Terms of Use.
Who we are
Axis is a Florida medical group. Our physicians and nurse practitioners provide specialty care (complex care, infectious disease, physiatry and pain management, podiatry and wound care) to patients in skilled nursing, assisted living, inpatient rehabilitation and long-term acute care facilities.
This policy covers the website, not your medical records
If you are or were a patient of an Axis clinician, your medical information is protected by federal law (HIPAA) and Florida law, and is described in our Notice of Privacy Practices. This Privacy Policy covers information you give us through the website, such as consult requests and job applications.
Please don’t send patient information through this website. Our forms are for facilities and job applicants. Please don’t enter patient names, dates of birth, room numbers or other details that could identify a patient. We’ll collect what we need securely when we follow up.
What we collect
- Consult requests from facilities: your name, facility, work email, phone number, the specialty you need, and your message.
- Job applications: your name, email, phone number, a short bio, your CV or resume, and anything else you choose to send (such as licensure, training and work history).
- Emails you send us: whatever you include.
- Information collected automatically: like most websites, ours receives basic technical information when you visit, such as your IP address, browser and device type, pages viewed, and the site that referred you. We also use the UserWay accessibility tool, which may store your display preferences in your browser.
Cookies
We use cookies that the site needs to work and cookies that remember your preferences (such as accessibility settings). You can block or delete cookies in your browser settings, but some features may not work properly. We don’t use advertising cookies or sell advertising on this site.
How we use information
- To respond to consult requests and arrange specialist visits with your facility.
- To review job applications, contact applicants, and verify licensure and credentials for anyone we hire.
- To answer questions and keep records of our business communications.
- To run, secure and improve the website.
- To meet legal obligations and protect our rights.
How we share information
We don’t sell personal information. We share it only as follows:
- With service providers who host our website and email, store job applications, and support our operations. We give them only what they need to do that work, and we expect them to protect it.
- With the administrative team that supports Axis. Axis receives administrative services such as scheduling, credentialing, human resources, billing and IT from a management company under written agreements. That team handles consult requests and job applications on our behalf.
- When the law requires it, for example in response to a subpoena or court order, or to protect the safety of patients, staff or others.
- In a business transfer, such as a merger or sale of our practice, subject to this policy.
- With your permission.
Emails from us
If you send a consult request or a job application, we’ll contact you about it. We may occasionally send facility contacts short updates about our services. You can stop those at any time by replying to the email or writing to contact@axismultispecialty.com. Any marketing email we send will tell you how to opt out. We won’t send marketing text messages unless you’ve agreed to receive them.
How long we keep information
- Consult requests: we move the details we need into our scheduling records and delete form copies from the website within 90 days.
- Job applications: we keep them for at least one year, as employment law requires, and generally no longer than three years unless you’re hired. If you’re hired, your information becomes part of your personnel and credentialing file.
- Website technical data: kept for as long as our hosting provider keeps it under its standard settings.
Security
We use reasonable administrative, technical and physical safeguards to protect the information we receive. No website or email system is perfectly secure, so please don’t send sensitive information, especially patient information, through the website or by regular email. If a security incident affects your personal information, we’ll notify you as required by law, including Florida’s Information Protection Act.
Your choices
You can ask us to tell you what personal information you’ve submitted through the website, correct it, or delete it (unless we need to keep it for a legal reason, such as an employment record). You can withdraw a job application at any time. Write to contact@axismultispecialty.com.
Children
This website is for health care professionals and facilities. It isn’t directed to anyone under 18, and we don’t knowingly collect information from children.

Our website may link to other sites. We aren’t responsible for their content or privacy practices.
Visitors outside the United States
Axis is a Florida practice. Information you send us is stored and processed in the United States.
Changes to this policy
We may update this policy. The effective date at the top shows when it last changed.
